About the course
What this course covers.
The course calls it incident analysis rather than investigation for two reasons, and both are worth taking on board. The word investigation can sound like an interrogation looking for someone to blame, which is the fastest way to make sure nobody tells the investigator the truth. And what a good investigation actually does is analyse an incident so corrective actions can be put in place. The goal is prevention, and the rule is fact-finding, not fault-finding.
Serious incidents cost organisations in morale, reputation, legal costs, lost production and lost bids, and the course starts by weighing those costs and risks. It explains the purpose and benefits of analysis and who should be involved, the stages an incident moves through, and the range of factors that can contribute: the work environment, job constraints, supervisor experience and worker experience. Incidents rarely have a single cause, and each factor has to be examined for the part it played.
The method follows from there: identifying root causes, gathering three types of evidence, developing corrective actions that address the causes, and writing and completing an incident analysis report. Throughout, the course stresses the three things that make an analysis worth doing: good information gathered and analysed, corrective actions actually taken, and those actions monitored for effectiveness. It is delivered as an animated story, which keeps a two-hour course moving.

Why it matters for compliance
Canadian OHS legislation requires employers to investigate serious incidents and to take corrective action, and certificate of recognition programs look for a working investigation process with trained investigators. Reporting obligations and timelines differ by province, so follow your own jurisdiction's rules alongside the method taught in this course.
Common questions
Questions buyers ask
Why is it called incident analysis instead of investigation?
- Because investigation can sound like a search for someone to blame, which discourages people from speaking openly, and because the real work is analysing the incident to prevent it happening again. The method is the same as a good incident investigation.
What are the three types of evidence?
- The course teaches how to gather three types of evidence and use them together to find root causes. Your own procedure may name them differently, but the principle of using more than one source is the same.
How much does the course cost?
- The course is 89.99 CAD per seat. Ask our safety experts for 10 percent off your first course, or a free On-Track Safety corporate account for 20 percent off every course for the first three months.
Does the course cover writing the report?
- Yes. The final section covers how to write and complete an incident analysis report.
How long does it take?
- A little over two hours, with testing throughout. It can be completed over several sittings.
Is there a shorter investigation course?
- Yes. Accident Investigation and Workplace Incident Investigation are shorter options. This course goes further into causes, evidence and the report.
Who should be on an incident investigation team?
- The course covers who could be involved in an incident analysis. Typically that includes the supervisor of the area, a worker familiar with the task, and often a health and safety committee member, with others added depending on the incident.
