Gap assessments and reviews

Safety program review after an incident or regulator order

Also called: Post-incident program review

After a serious incident or an OHS order, we review the parts of your safety program that should have prevented it. You get an independent account of what failed in the system and a practical corrective action plan, ranked by what matters most.

Who does the work: Building and auditing safety programs for companies across Western Canada since 2008.

Who asks for a post-incident program review?

After a serious incident

Your investigation found the immediate cause. You need to know what in the program let it happen.

After an OHS order

An officer has written orders. You need a credible plan and evidence that it is working.

Boards and owners

Leadership wants an independent view, not the program reviewing itself.

What do we check?

The program around the incident

  • Hazard assessment and controls for the task involved
  • Training, competency and supervision for the people involved
  • Inspections, maintenance and earlier warning signs

The response

  • Whether the investigation reached the system causes
  • Corrective actions: specific, owned, dated and checked
  • What the orders require and the evidence to show compliance

What does the report look like?

Four pages from a sample report, built on invented data. Real reports follow the same order: the summary first, then the scores, every improvement in full, and a roadmap to close them.

Sample gap list

What does a gap look like?

Every gap names what we found, the evidence behind it, the fix, and the reference it comes from. Filter the sample list or open any row.

Showing 8 of 8 sample gaps

  • Evidence we checked
3 of 5 task assessments sampled pre-date the change; the night crew supervisor confirmed in interview.
    The fix
Review the three assessments with the night crew, add lighting and fatigue controls, and set a review trigger for any change in hours.

How does it work?

  1. Scoping questions

A short questionnaire of facts: sites, headcount, fleet size, who your customer is and what they asked for. We never ask "do you comply". That is our job to find out.

  2. Secure upload link

You get a private link with a named list of the documents we need, so nobody guesses what to send. Upload from any device.

  3. Desk review before we arrive

We read every document before the site visit, so field time is spent checking your records against the work, not reading binders.

  4. Field day

We walk the site, talk with workers and supervisors, and check what the records say against what is actually happening.

  5. Scored report and debrief

A scored report with a prioritized gap list, the reference for each gap, and a practical fix. We walk you through it and answer questions.

What does it not cover?

  • Not the legally required investigation. Your own investigation still has to be done; we review the program around it.

  • Not legal advice. Where there is a prosecution risk, we work alongside your counsel.

  • No promised outcome with the regulator. We help you build a credible plan and the evidence behind it.

Frequently asked questions

How is this different from an incident investigation?

The investigation finds what happened and why on the day. This review looks at the safety program around the incident, such as hazard assessment, training, supervision and inspections, to find what let it happen and what else might be exposed.

Can you help us respond to an OHS order?

Yes. We review what the order requires, check your program against it, and help you build the corrective action plan and the evidence that it is in place.

How quickly can you start?

Usually within days. Tell us the deadlines in any order you have received and we will plan around them.

Book a post-incident program review

Tell us what your customer, regulator or certifying partner is asking for. We will scope it and quote it per site, usually the same business day.
